Every time I've seen this one it's been the motherboard.
I've had similar problems with mice and I've changed the mouse for a
Microsoft (yuk) one. They work every time. Other than that I don't
know what to suggest ? The ATI cards do work well with XF86Setup. I
know that because I've done a few. I had to select generic ATI card
(I think).

See... http://www.uk.debian.org/~phil

or mail to phil [at] hands.com. He's the Debian expert I met at the Linux
Expo in London.
