This ain't perfect, but your best bet is Ghostscript and the extract
text option. You will get all the text, but the formatting is lost, and if
it is in columns, then a lot of reorganising text is required. I'm just
doing that for the Bash ref card, but a bit of re-organising is
required, but its quicker than typing it all out again...
