Strictly speaking, I don't think it needs SMTP, although that's the
preferred way to do it (and by far the easiest).  IIRC, you can
actually feed directly from fetchmail into a local MDA such as
procmail or deliver via a pipe.

It definitely doesn't need sendmail, any smptd (qmail, exim, etc) will
do.
