Thats the same config as mine. You need something to translate buttons 4 and 5 to keypresses, tho', and thats where imwheel comes in, unless you run a windowmanager with direct support (icewm, and possibly KDE 2? I don't actually know)
Also, I would imagine it depends on the graphics driver. It seems to have some troubles when updating large portions of the screen (like scrolling a maximised browser window) so it could in fact be something to do with that?
Actually, I just found that xmms jumps if I scroll with the mouse in xfmail, but not if I scroll with the keyboard

I'm running XFree 4.0.3 on my RH7.1 box, and 4.0.1 on the RH7.0 boxen
without any hassles.
The mouse config in /etc/X11/XF86Config-4 is

It's the ZAxis bit that deals with the middle wheel. I tried using
IMWheel with 3.3.6 and it was a nightmare - config files just to get
Netscape to do it's thing right etc...
XMMS doesn't skip a beat, both on my folk's machine (RH7, X4.0.1, 64Mb) and my
own (RH7.1, X4.0.3, lots of Mb) when scrolling, opening windows, etc.
I wouldn't say that the RH XFree setup is exactly tuned to perform...
