If you bring up the connection on demand from the local machine, are
other machines on the LAN then able to access the internet? If not, the
problem isn't with the demand script at all - ip forwarding won't be set
up correctly.

If they can connect to the LAN, have a look at the man page for smpppd
and see how it says it detects network activity - it might be that it
blocks traffic from the local LAN by default to prevent the link being
brought up accidentally (I don't have the smpppd software installed, or
I would look for you).
