Well the first thing that springs to mind is if SMART_HOST is a name, and
there's nowt in /etc/hosts for it, it'll fail to send the email :) Ditto for
LOCAL_RELAY I'd guess.

How I configured it at work (this may work for qmail as well, but I think its
a little more picky as it has no support for /etc/hosts): relay all mail
through ISP's external mail hub ... put entry in /etc/hosts that resolves the
relay's host name to an IP :)
Now with qmail there is control/smtproutes, but I've never tried putting a
simple entry in like:
:[11.22.33.44]
(where 11.22.33.44 is the IP of the relay). IIRC, the square brackets tell
qmail that it's dealing with an IP, so don't bother doing trying to resolve
it :) In theory it could work, but I've never had the need to try this in
practice - also got DNS available and send direct avoiding the ISP's mail
servers (been burnt by them in the past to trust them).

exim I looked at about five years ago ... never got it working under linux.
It probably has improved much since then :) But the hassle I had got me
pointed to qmail instead. Postfix I config'd for work as we don't use
internal DNS (which is really annoying but I'm not going into the why's and
wherefore's ... but it will be introduced eventually, even if its through the
back door).
