Yep. I got one with a .cgi last time I did it, but I guess it's the same
file.

Depends how you're trying to do it. Your distro config tool would be the
first port of call probably - yast should be able to set up wvdial for
you, most of the other distros have similar stuff. I believe even debian
has a ppp setup tool? ;) 

Most of the stuff is fairly obvious. Username, password, etc. You'll need
to know where your modem is, obviously. Some things you might not be able
to fill in, usually this won't matter or there'll be an 'auto' setting.
