Going from meaty to simple...

Qmail with serialmail (both at http://cr.yp.to/) - Qmail is not really
that hard to set up, I haven't used serialmail - but most of
D.J. Bernstein's stuff is easy enough to get going.

Sounds like nullmailer does the job too
(http://untroubled.org/nullmailer/) - it's designed to relay to a
fixed set of smart hosts - i.e. your ISPs mailservers for relaying to
the destination.

I use getmail (http://www.qcc.sk.ca/~charlesc/software/getmail-2.0/)
instead of fetchmail.

Getmail delivers messages directly to mbox or Maildir files - it does
not fetch them from a POP box and inject them into a local mailserver
queue for local delivery.

Of course, set $FAVOURITE_MUA to fetch from remove POP boxes and send to
remote SMTP mailservers saves any of this - but that wouldn't be any
fun, now, would it? ;-p
