I'm not quite sure exactly what you're trying to do, my MySQL is fairly well 
documented - the manuals are at the MySQL site (mysql.org?) and there are 
probably manpages too.

PostgreSQL is another alternative if you're looking for a full SQL database.
Oracle 8i is also available for Linux - you'll still need to pay for licenses 
though.

As far as creating forms goes, the only experience with that I've got is using 
AOLserver and Tcl to interact with an Oracle database, or Apache and PHP to 
talk to a MySQL database, both resulting in dynamic webpages with information 
from the database. (Oracle was on both Solaris and Linux, MySQL on Linux)

Dunno if that helps, but I thought I'd give my 2p.
