It is on the install media in /disks/rescue.bin (IIRC)

You can just dd it to a floppy disk. You could also use the CD itself as
a rescue floppy?

If you want a general purpose boot floppy, try TomsRTBT. It can't handle
reiser file systems, but other than that it is pretty spiffy.
