We'll have to find out from the NHS.  As I understand it they are told 
that they will pay a yearly subscription.

Yes.  I think my gut reaction is to think along the lines that since 
the NHS have stuck themselves with "the best tool for the job" it 
might be that they would rather live in the same trench forever more 
?  It's easier to live on the farmers chickens in the trench than be 
the farmer and run the farm - or change the source code for your own 
use.
