Sounds like it's running a single application (radiotherapy reporting
web interface?). If so then that app will need only a small percentage
of the installed packages, the rest will be redundant.

In this case you may be best off with a minimal initial install. Rather
than blindly applying updates automatically, apply updates at say
6-monthly or yearly intervals if there has been some package update that
you think you need. Or install it, get it running nicely, put on a
firewall to keep out any internal intruders, then don't worry about
updates (tho keep an eye for important security updates that would
affect your set-up).
