It's straightforward to see if PulseAudio is the problem by trying it directly 
with ALSA. You can change the output of gstreamer programs (Rhythmbox, Totem 
(without Xine), etc.) by running gstreamer-properties. To stop ALSA itself 
running through PulseAudio you may also have to comment out or move the files 
/etc/asound.conf and ~/.asoundrc .

With things configured without PulseAudio then you can kill the daemon with 
pulseaudio -k then try something gstreamer based. If it works then PulseAudio 
is at fault and you'd probably be best without it at the moment, if not then 
you may have an ALSA issue, which can be more annoying to figure out, but 
there's thankfully a lot more Google hits for them :)
