Have you looked in /usr/doc/qmail-1.03 /usr/doc/serialmail-whatever?
Sometimes the readmes are pretty good...

Outbound mail will need the smarthost setting configured, not sure about
the authorization problem. It sounds as if you haven't got the correct
server, or the correct login information though. Might be worth contacting
your ISP for the correct settings?
