I'm with Entanet which I got via the UK Free Software Network
(http://www.ukfsn.org/). It ticks all your boxes I think (just checked and
there's an 020 number on their website). When I asked BT about a static IP
address they put me on to their business account manager and started
talking about £100 per month. I'm currently paying £20 a month (I think).
I think  a BT line is needed and there's been very little downtime.
